Cancellation Policy

Please note that because Dr. Dworetsky holds an appointment time specifically for YOU - you will be charged for any appointment missed or not cancelled at least 24 hours in advance of your appointment time. Insurance companies do not reimburse for missed or cancelled appointment fees and you will be solely responsible for the charge.

We do accept credit and debit cards for your payment convenience.

Patient Forms

It is extremely helpful for the office to have a copy of the front and back of your insurance card. If your insurance information changes, please make sure to let us know. If you are a new patient, recently moved, your health insurance has changed, or need to sign a release of information, just below are copies of the office forms. Please feel free to download them, print them out, and complete them prior to your next visit.

Please bring to the first appointment: a list, as comprehensive as possible, of all current medications (name and milligram strength) as well as a list of all previous psychiatric medications tried (name, milligram strength and side effects that caused the medication to be discontinued) as this will definitely speed up the initial evaluation.

It makes it easier if we can have a credit/debit card or HSA/FSA account to automatically charge co-payments or balances. A "credit card payment form" can be found below to create a method of automatic payment with my office and thereby reduce invoices, stamps and paper. A receipt of automatic payment will be sent to you every time your card is used.
